Completing the Waiver and Release for High Risk and Hazardous Activities authorization below allows your child to participate in courses that may be considered high risk. Your EF will not be able to place order requests for high risk activities unless this form has been signed by both the learner and parent. You will receive a copy for your records upon submission.
High Risk and Hazardous Activities include: All Sports, Trampoline/Parkour, Cooking, Makery, Woodworking, Metalwork, Industrial Arts, Science Activities, Bunsen Burner Use, Animal Bioscience, and other similar activities to be discussed with your EF.
